Asthma Manager

Asthma Manager is our key product. It is Health 2.0 software with an interface for both patients and healthcare professionals.

Patients benefit from a mobile/tablet app which provides an interactive personalised care plan and enables self-management of their asthma.

Asthma Care Plan

GPs and asthma nurses have a web-based tool, accessed through SystmOne, which provides the framework for a structured asthma assessment and clinical decision support based on the latest BTS/SIGN asthma guidelines.

Background

The company has evolved from the meeting of a software developer with an architectural background and a GP with an interest in IT. The result is well designed software with a sound clinical background.

Our work has been nominated for awards by the NAPC (National Association of Primary Care) and Medipex.

Pharmacy

A version of Asthma Manager is available for Pharmacies.

Poor inhaler technique and patient misconceptions about medication are recognised factors in poorly controlled asthma. The focus of this version is correct inhaler technique and personalised medicine education.

The software can be used in store as part of a Medicine Use Review and to support asthma reviews as part of the Quality Payments scheme.

A link is provided to patients to access their medication information on a mobile or tablet. As well as benefiting patients, this encourages loyalty to the pharmacy.

NHS Commissioning

Asthma affects around 5.2 million people in the UK*. Poorly controlled asthma has a major impact on patients’ quality of life as well as significant health costs.

Asthma Manager software can contribute to improved asthma management in primary care by promoting adherence to guidelines and national or local prescribing formularies.

A integral part of the software is the provision of a patient care plan, which has been shown to reduce the incidence of unplanned hospital attendance or admission. Around two thirds of asthma sufferers do not currently have a care plan.

The care pathway provided by Asthma Manager supports the aims of ‘New Models of Care’

The license cost is based on patient list size. Discounts can apply for multiple licenses purchased by CCGs. Please contact us for more information.

In Development

We are working on apps for other chronic diseases including diabetes. The aim is to support healthcare professionals in providing better quality care by making the data within primary care systems more accessible as well as giving patients a platform to interact with their health records.

Open Source

Active Medicine is a member of InterOpen.

We support the principals of Open Source particularly the development of APIs for Electronic Health Records. The patient’s data belongs to the patient!
